Name of the animal you are interested in and why you would like to offer them a home:

Please describe where you live. Do you own or rent? Is it a house or apartment? Do you have a securely enclosed garden?

Who lives at your property and how old are they? Do all the family agree to a new pet? Please include yourself, all other people and animals.

Please give brief details of what work you and other adults in your household do. How many hours per day will the animal be left alone?

Where would the animal live within your home? Where would it sleep?

What experience do you have with pets previously? Number of years, type of pets etc.

Are you willing to spend time training your dog(s)? Please give brief details of proposed training methods.

Are you willing to walk your dog for at least 20 minutes every day or give them suitable alternative stimulation if public walking is not suitable for this dog?

Are you willing to send or email us photos and updates on your pet for the remainder of their life?
